A comparison of the efficiency of three different phacoemulsification fluidic systems for cataract removal

Abstract Details

Purpose:

We compared three different phaco machines with three different infusion systems (gravity infusion system versus combined Gravity-pressurized infusion system and Active Fluidic with plate compressed soft bag) and two different kind of aspiration pump (Two peristaltic and a Venturi pump) in order to evaluate the role of fluidic on the efficiency in cataract removal. The efficiency has been evaluated monitoring the time of foot pedal in position 3 (ultrasound-on) to remove the cataract.

Setting:

Department of Ophtalmology , Clinica Mediterranea Naples - Italy

Methods:

Fortyfive patients have been operated at Clinica Mediterranea Naples - Italy by the same experienced surgeon and randomly divided in three groups to be operated with three different phaco machines. Group 1 Signature PRO (AMO), Group 2 Stellaris Elite (Baush e Lomb) and Group 3 Centurion (Alcon). The cataract density grade was evaluated after the surgery by the surgeon and divided into 4 stages (Stage 1 soft, stage 4 hard rock). The time of “foot-pedal position 3” was registered by using the report of the phaco after each surgery in order to compare the energy delivered for cataract removal.

Results:

The analysis compares the ultrasound time necessary for the cataract removal with the three different phaco machines. The average cataract grade is almost the same in the three groups. The Alcon required the 45,9% of US less respect to the AMO J&J pro and the 65,2% less of US respect to the B&L to remove the cataract. ANOVA analysis of variance followed by Bonferroni Test were used for the statistical analysis to compare the three groups. Alcon showed a statistically significative reduction in time of Ultrasound respect to AMO e B&L to remove the cataract (P< 0.05).

Conclusions:

Data analysis demonstrates that the fluidics plays an important role in the efficiency of the phaco-machine and influences the energy delivered to remove the cataract. The typology of infusion system together with the aspiration pump combine to best balance the anterior chamber stability during the surgery and to reduce the necessity of ultrasounds to remove the cataract. Our study shows that the combination of Active Fluidics with the soft bag compression and the 7-roller peristaltic pump (Centurion) best reduces the energy delivered for the cataract removal. This is useful during cataract removal and for a rapid vision restore after surgery.
