Clinical research of posterior capsule rupture in phacoemulsification by trainee surgeons

Purpose:
To investigate the case and prognosis of posterior capsule rupture (PCR) in the surgery trainees’ phacoemulsification.
Setting:
Qingdao Xinshijie Eye Hospital, Qingdao, China.
Methods:
Retrospective cohort study. The first 200 cases performed independently by 6 surgery trainees from August 2016 to December 2018 were enrolled in this study. All the cases were divided into two groups: the first 100 cases group and the last 100 cases group. The occurrence rate of PCR, phaco or irrigation/aspiration (IA) phase, situation of IOL implanting, retinal detachment, dropped nucleus and postoperative visual acuity were collected and analysed.
Results:
In the 1200 cases, 39 cases (3.25%) underwent PCR. Compared with the last 100 cases group, the occurrence rate of PCR and vitreous loss were higher in the first 100 cases group (P=0.015, P=0.012). At the same time, PCR was more likely happened in the phaco phase in the first 100 cases group and in the IA phase in the last 100 cases group (P=0.017). There was no difference between the two groups about the situation of IOL implanting, retinal detachment, dropped nucleus and postoperative visual acuity.
Conclusions:
With supervised surgeon and suitable cases, the occurrence rate of PCR of surgery trainees could be controlled to the desired level. However, it is still need a long process to manage the PCR and complex cataract surgery independently for the surgery trainees.
