Bilateral endophthalmitis after bilateral simultaneous cataract surgery: case report

Abstract Details

Purpose:

To report a case of bilateral endophtalmitis due to Stenotrophomonas maltophilia(SM) after bilateral simultaneous cataract surgery ( BSCS)

Setting:

15th  september

Methods:

Clinical data include medical history, complete laboratory blood analysis, antibody of Borellia burgdorferi, anticardiolipin, lupus anticoagulans, lupus cells, quantiferone cell,  lumbal puncture, MR. Ocular data including visual acuity , ultrasound, OCT of disc and macula , FFA , result from culture

Results:

38 year old woman underwent uncomplicated BSCS.Symptoms began 5 day on her left and 12 days on her right eye after surgery. Medical therapy local and intravitreal ( antibiotics, antimicotics, steroid ) and vitrectomy with lavage of the anterior chamber and capsular bag were insufficient to treat the infection . Despite current treatment there were episodes of remission and recurrence. Vitrectomy with IOL extraction and capsulectomy were performed to complete remission and send for laboratory analysis. Stenotrophomonas maltophilia was isolated . Visual acuity has remained stable for 9 months, without signs of inflamation

Conclusions:

In case of post cataract surgery endophtalmitis  Stenotrophomonas maltophilia should be considered. Vitrectomy with IOL extraction may contribute to better clinical outcomes  especially when infection is refractory to other medical treatment .
