The management of small pupils in complex cases of cataract surgery

Abstract Details
Purpose:
To describe an easy and safe way to manage challenging cataract surgery cases, such as small pupils, typically seen in pseudoexfoliation syndrome and in traumatic cataracts.
Setting:
Eye Unit, Rimini Hospital; University of Bologna, Italy
Methods:
Ten cases of complex cataract surgery in small pupils were performed using a peristaltic pump phacoemulsification system. In all cases, a high-viscosity cohesive ocular viscoelastic device (OVD) and a Malyugin ring pupil expander (Microsurgical Technologies, Bellevue, WA, US) were used.
Results:
All ten cases underwent phacoemulsification and posterior chamber IOL implantation without any major intra- or post-operative complications, such as posterior capsule rupture and nucleus or cortical fragments luxation in the vitreous chamber, with no need for further surgical procedures.
Conclusions:
Proper pupil dilatation is fundamental to perform a safe and controlled cataract surgery procedure especially in complex cases with small pupils, such as traumatic cataracts or in pseudoexfoliation syndrome. Malyugin ring pupil expander is probably the best option to achieve an optimal and regular pupil expansion without significant adverse effects and surgical complications in the hands of well-trained surgeons.
