Nd:YAG laser treatment of capsule contraction syndrome associated with Irvine Gass syndrome

Abstract Details

Purpose:

To present a patient with severe   Capsule contraction syndrome (CCS) and pseudo phacic cystoid macular edema (Irvine Gass sebdrome =IGS)

Setting:

Department of Ophthalmology, Ulucanlar Eye Education and Research Hospital, Ankara, Turkey

Methods:

A 58 year women presented to ophthalmology clinic complaining with blurred vision one month after phacoemulsification surgery on her right eye.  She had underwent complete ophthalmological examination. Best Corrected Visual  Acuity (BCVA), IOP measurement, fundus evaluation and OCT examinations were done for both diagnosis and during follow up period after treatment.

Results:

She had  severe CCS with  total closure of capsular opening (CO) on slit lamp examination.  Her VA was hand motion on effected side. Circular shape  Nd- YAG laser capsulotomy was performed  to open total closure of CO.   Free flab was enter to anterior chamber (AC). Two weeks later her BCVA improved to 0.1 without any reaction and residual material in  AC. On fundus examination and OCT   evaluation cystoids macular edema (IGS) was diagnosed. Intravitreal bevacisumab  injection was done. One month later her BCVA improved to 10/10 and anterior capsular opening was remained without any reaction on both AC and fundus.

Conclusions:

Severe CCS  with total occlusion of capsular opening  can be presented in post operative one month period. ICG may  associated with CCS or  Nd- YAG laser capsulotomy  can   treated with intravitreal bevacisumab injection.
