Purpose:
To present two cases of retinal toxicity after intracameral injection of a high dose of cefuroxime at the end of cataract surgery
Setting:
Department of Ophthalmology, Istanbul University, Istanbul Faculty of Medicine, Istanbul, Turkey
Methods:
Two cases developing toxic retinopathy after uneventful phacoemulsification were observed. Both patients erroneously received a high dose of cefuroxime for endophthalmitis prophylaxis at the end of cataract surgery. Both patients had compromised zonular integrity.
Results:
At one day post-op, distance visual acuity was count fingers at 1 meter. Funduscopy revealed decreased foveal reflex and pigment epithelial changes. Spectral-domain optical coherence tomography revealed loss of outer retinal layers (loss of ellipsoidal zone) and fundus autofluorecence revealed diffuse hyperautofluorescence in both cases. Despite treatment with anti-inflammatory medications, no changes were observed in visual acuity or fundus findings at 1-year follow-up.
Conclusions:
Toxicities induced by medications or substances introduced into the eye during surgey should be considered in patients with no rise in visual acuity postoperatively. Multimodal examination techniques are helpful in making the diagnosis.
