Safety and efficiency of phaco surgery in various types of post-uveitic cataracts

Abstract Details

Purpose:

Purpose: To asses and evaluate the safety and efficiency of phaco surgery in various types of cataracts surgery

Setting:

Venue: Prizren Regional Hospital Eye Ward and American Vision Eye Center, Prizren

Methods:

Methods: A total of 57 eyes of 45 patients with post uveitic cataract were operated with temporal clear corneal incision in a period of 30 months between January 2016 until June 2018. This study included various types of post uveitic cataracts with occluded pupilae, festooned pupils, non dilating pupils, post trabeculectomy uveitic cataracts, , matured hard uveitic cataracts.. Standard preoperatice steroid drop and systemic corticosteroid regimen was applied in all patients week before surgery. Most of the cases were performed under topical anesthesia, only 7 cases under general anesthesia, by single surgeon.

Results:

Results: Intraoperative complications were negligible with the incidence of AC instability {4.22 %}, descemet stripping {1.45 %},, floppy iris {25%}, uncontrolled rhexis formation  {2,87%}, and linear imprints at the papillary margin due iris retractors {39.5%}. Postoperative complications included: day one postoperative corneal edema {40.7%}, fibrinous reactions {0.87%}, increased IOP {7%}, cistoid macular edema {11.4 %}, papillary capture {1.45%}. More complications were observed in hard uveitic cataract cases. A high percentage {61 %} have recorded BCVA more than 6/12 or better.

Conclusions:

Conclusion: Adequate exposure of papillary are using iris retractors makes phacoemulsification safe and effective in theses hard cases and helps overcome unavoidable complications
