Subjective gradation system of posterior capsule opacification intensity: repeatability and reproducibility

Abstract Details

Purpose:

To evaluate the repeatability and reproducibility of gradation system of posterior capsule opacification (PCO) intensity using slit-lamp pictures in patients implanted with two different intraocular-lens (IOL) platforms: Tecnis and Clareon.

Setting:

Miguel Servet University Hospital, Zaragoza, Spain.

Methods:

Different slit-lamp pictures were taken by the same experienced technician using the same method, but only the best one was selected. Three different masked ophthalmologists graded the intensity of PCO according to the following scale: 0 means no PCO, 1 means soft and non-delimitable, 2 means delimitable plaque, 3 means hard plaque and 4 means completely opaque. The three ophthalmologists graded all the pictures three different times in different days and without knowing the gradation of the others. The pictures were taken 1 and 6 months after cataracts surgery. Intraclass correlation coefficients (ICC) were calculated.

Results:

Regarding reproducibility of the pictures in the first month, the ICC was 0.82, 0.80 and 0.77. In the pictures at the sixth month, the ICC was 0.83, 0.76 and 0.78. Regarding repeatability, the first evaluator achieved an ICC of 0.92 and 0.95 in the pictures of the first and sixth month, respectively. The second evaluator achieved an ICC of 0.95 and 0.88 in the pictures of the first and sixth month, respectively. The third evaluator achieved an ICC of 0.86 and 0.93 in the pictures of the first and sixth month, respectively.

Conclusions:

Despite of some differences, this subjective gradation system of PCO intensity achieves very high repeatability and a good reproducibility. It is an easy scale which allows and adequate quantification of PCO with experienced evaluators and technicians.
