Assessment of posterior corneal changes after accelerated corneal cross-linking for keratoconus

Abstract Details

Purpose:

We aimed to evaluate posterior corneal curvature changes after crosslinking treatment for keratoconus patients in terms of corneal refractive power and asphericity.

Setting:

University of Health Sciences Prof.Dr.N.Resat Belger Beyoglu Eye Training and Research Hospital, Istanbul Retrospective design

Methods:

Patients’ records whom applied our clinic due to keratoconus between January 2017 - December 2017 and underwent accelerated corneal crosslinking were retrospectively reviewed. 51 eyes of 39 patients with at least 1 year postoperative follow-up records and Scheimpflug and placido-based corneal topographic examination (Schwind Sirius) results  were included in the study. In addition to demographic data, preoperative and postoperative best corrected, uncorrected visual acuity, manifest refraction values, cylindrical powers, spherical equivalent values and corneal topographic values of the posterior cornea, keratometric power values and aspheric values (Q value) in the 4.5 and 8 mm zone, posterior elevation values were compared.

Results:

The mean age was 22.3 ± 5.4 years (10-36). The mean follow-up period was 14.3 months (12-19). The posterior corneal keratometric value in the 4.5 mm zone and 8 mm zone were  -7.9 ± 0.9 D;  -7.7 ± 0.9 D preoperatively and  -8.1 ± 1 D and -7.8 ± 0.8 D postoperatively (p: 0.002, p: 0.049). Asphericity was -1.75 ± 1.3 in 4.5 mm zone, and -1.4 ± 1.0 in 8 mm zone preoperatively, in the postoperative period the changes in asphericity were not significant. (p: 0.809, p: 0.504). There was a significant increase in posterior elevation postoperatively (p: 0.007).

Conclusions:

Posterior corneal changes can be detected after at least one year follow-up, even in eyes that are thought to be stable after corneal cross-linking treatment.
