Comparing keratoconus and post-LASIK ectesia progression rate after corneal collagen cross-linking: 3 years' follow-up

Abstract Details

Purpose:

To report and compare the rate of ectesia progression between patients with keratoconus and post-ectesia over 3 years post crosslinking (CXL).

Setting:

Beirut Eye&ENT Specialist Hospital

Methods:

a retrospective cohort study of patients undergoing CXL treatment for either keratoconus or post- LASIK ectesia ( referred as ‘’ectesia’’). Visual acuities, manifest refraction, keratometry and pachymetry were followed and compared between stable and patients that are progressing in each group. Also, analysis was performed to identify the difference in the linear progression over 3 years between progressing keratoconus patients and patients with ectesia. 54 eyes (31 patients) with ectesia and 111 eyes (67 patients) with keratoconus were included in the study

Results:

12 out of 54 eyes with ectesia (22.2%) and 4 out of 111 eyes (3.6%) with keratoconus had a progression post CXL (p-value: 0.0001). Mean keratometry increased after a drop at 6 months post CXL in progressive keratoconus patients while maximum keratometry increased after a drop at 6 months in ectasia. Using the linear model to compare the best corrected visual acuity, spherical equivalence, sphere cylinder, mean and maximum keratometry and pachymetry between patients with progressive keratoconus and progressive ectesia (p>0.05), Also, subgroup analysis excluding patients who received PRK and ICRS showed insignificant difference between keratoconus and ectesia recurrence

Conclusions:

Eyes with post- LASIK ectesia seems to be less responsive to crosslinking than keratoconus.
