Abstract Details

Purpose:

To subjectively assess range of vision in patients implanted with aberration-neutral, hydrophobic acrylic monofocal (MX60E, Bausch & Lomb) intraocular lens (IOL) technology when used to correct aphakia following cataract extraction.

Setting:

Single-centered, single-surgeon, prospective clinical case series

Methods:

A single-center, single-surgeon, prospective case series involving 117 eyes of 62 patients (55 bilateral) with age-related cataract without ocular co-morbidities and treated with standard cataract extraction.  Patient outcomes at 1 month were analyzed to determine uncorrected distance and intermediate visual acuities (UCDVA and UCIVA), manifest refraction spherical equivalency (MRSE), absolute prediction error (APE), assessment of UCDVA relative to MRSE, and subjective questionnaire (modified NAVQ).

Results:

At 1M, mean UCVAs (logMAR) were 0.11±0.18 for far and 0.14±0.13 for intermediate.  Majority of patients (75.2%) were seeing 0.2 logMAR or better at both distances.  Mean MRSE was -0.26±0.57D with APE of 0.05D, 70% of patients were within ±0.50D of refractive target.  Comparative analysis of UCDVA to MRSE diverged from theoretical defocus of 1-line reduction in VA per -0.25D of uncorrected MRSE.  LogMAR values by residual MRSE were 0.05 (0.00 to -0.25D), 0.109 (-0.26 to -0.50D), 0.19 (-0.51 to -0.75D) and 0.29 (-0.76 to -1.00D), respectfully.  Most patients (≥60%) had little to no difficulty seeing computer or mobile phones.

Conclusions:

Patients implanted with aberration-neutral hydrophobic acrylic monofocal IOL demonstrated expanded range of vision with little to no difficultly with intermediate tasks.  Accurate refractive targeting was achieved post operatively with minimal aim error.  The platform’s capacity to provide tolerance to myopic defocus may allow surgeons more flexibility in targeting for an expanded range of focus without sacrificing distance acuity.
