Postoperative analysis of glistenings and posterior capsule opacification (PCO) after phacoemulsification: comparison of two different intraocular lenses

Abstract Details

Purpose:

to determine the  incidence of glistenings and posterior capsule opacification ( PCO) and the relationship between the EnVISTA (Bausch&Lomb) and Acrysof  IQ (Alcon) intraocular lens types over 3 years.

Setting:

University Hospital Center Zagreb, Department of Ophthalmology, Zagreb, Croatia.

Methods:

Prospective study included 60 patients over a 1 year of recruitment.  Phacoemulsification with IOL implantation was performed in all patients. In 30 eyes EnVISTA IOL was implanted and in 30 Acrysof  IQ  IOL. Postoperative results of  1 month, 1, 2 and 3 years were compared. Glistenings was quantified with an  Image J software. The grading scale was, in order of severity, none,  mild, moderate and severe. Digital retroillumination images were taken of each eye. PCO values were assessed using a photographic image analysis system (EPCO 2000) . The rate of  neodymium:YAG capsulotomy for each IOL was also compared.

Results:

No glistenings were reported in EnVISTA group.In the Acrysof  IQ group 71 % eyes had mild, 27 % moderate and 4% severe glistenings. The mean PCO score was 2.78 ± 1.55 (SD) for the  EnVISTA and  2.32 ± 1.20 for Acrysof IQ group (P=.229). There were no significant differences in the median EPCO values in the entire IOL optics area  and in the central 3-mm optic zone  at 3 years. Three years postoperatively, Nd:YAG capsulotomy was performed in 28% of eyes with EnVISTA IOL and 23.33% with the Acrysof IQ  IOL (P=.692).

Conclusions:

Both IOLs had comparable PCO and Nd:YAG rates 3 years postoperatively. There were statisticaly significant difference between two IOLs regarding to glistening formations.
