Abstract Details

Purpose:

To present a case of a patient with disability glare due to calcifications on the back surface of the intraocular lens (IOL) due to asteroid hyalosis.

Setting:

Amphia hospital, Breda, the Netherlands

Methods:

Visual acuity was assessed, straylight measurements were performed with the C-Quant instrument (www.oculus.de) and slit lamp photographs were obtained. Measurements were repeated after treatment of the calcifications on the IOL’s posterior surface with a Nd:YAG-laser.

Results:

The patient had blurred vision in his right eye (RE), especially when looking in headlights. He was pseudophakic and had laser capsulotomy 2 years earlier. His CDVA was 0.7. Calcifications were observed on the back surface of the IOL; asteroid hyalosis was present in the vitreous. Straylight measurement was 2.08 log(s) – 8.2x higher than average for a pseudophakic eye. After polishing the back surface of the IOL with a Nd:YAG-laser, he subjectively regained good functional vision, also at night. His UDVA was 0.9+; his straylight improved to 1.76 – 1.3x higher than average.

Conclusions:

Asteroid hyalosis has been reported to induce calcifications on the back surface of the IOL after laser capsulotomy and lead to a reduction in visual acuity. We have shown that these opacifications may also induce clinically significant glare. Treatment with a Nd:YAG-laser can reduce the opacifications on the IOL and significantly improve complaints of disability glare.
