Early results of phacoemulsification combined with micropulse cyclodiode laser in patients with glaucoma: efficacy and safety

Abstract Details

Purpose:

To asses for the efficacy and safety of cataract surgery combined with MicropulsE transcleral cyclodiode (MP-TSCPC)laser in patients with co-existing cataract and glaucoma.

Setting:

St Thomas' Hospital, London, UK.

Methods:

This is a retrospective longitudinal study comparing the change in intra-ocular pressure(IOP),visual acuity(VA), the number of antiglaucoma agents used by the patients at baseline and three months follow up and the complications associated following the cataract surgery with MP-TSCPC. data was extracted from the medical notes of the patients. Data was analysed using paired samples t-test in SPSS.

Results:

21 patients had 3 months follow up. The mean baseline IOP was 19.43±4.85mmhg and 13.2±5.43mmhg at 3 months. 32%IOP reduction was achieved. The mean IOP reduction is 6.24±1.51mmhg (95% CI 3.08, 9.4)(p=0.001).The mean number of antiglaucoma agents at baseline is 2.62±1.24 and 1.95±1.11 at 3 months.The mean reduction of antiglaucoma agents is 0.66±0.73 (95%CI 0.33,0.99)(p=0).The overall mean Visual acuity at baseline was 0.21±0.07(Logmar) and 0.2± 0.07(Logmar) at 3 months.The change VA is 0.0047±0.2(Logmar)(95% CI (-0.09, 0.09)(P=0.9).At 3 months follow up:3 patients had CMO,2 patients posterior synechiae,4 patients persistent mild anterior chamber inflammation and 2 patients moderate AC inflammation.

Conclusions:

This is the first ever study assessing the efficacy and safety of this combined procedure. It shows that the combined procedures reduced IOP significantly at 3 months with corresponding reduction in glaucoma medication use and also improved VA. However, a longer follow up period is essential to assess the long-term changes.
