Abstract Details

Purpose:

To demonstrate the option of corneal wavefront guided transepithelial phototherapeutic keratectomy to achieve visual rehabilitation in eyes that suffered loss of vision after a corneal event.

Setting:

Bergman Clinics Netherlands, private practice for refractive surgery

Methods:

Various cases were reviewed that suffered from loss of vision due to anterior stromal irregularities after a previous corneal event, such as contact lens induced ulceration, ulceration after refractive surgery, irregularities after SMILE surgery, irregularities after RK surgery. The depth of the corneal lesion was measured with high resolution corneal OCT (Zeus, CSO). The highly aberrated corneal state was treated using corneal wavefront guided Transepithelial PRK (Schwind Amaris) where the PTK phase was adapted according to the depth required to remove the lesion.

Results:

In all cases the ablation depth respected the residual stromal bed thickness as defined in international refractive surgery standards. In all cases a tremendous reduction of high order aberrations and improvement of uncorrected visual acuity was achieved.

Conclusions:

Anterior stromal irregularities with a great variety in ethiology can be corrected adequately with a one-step surface ablation that simultaneously treats high and low aberrations and removes the lesion, yet respects the minimal residual stromal bed limits.
