A prospective evaluation of topography-guided LASIK surgery: preliminary results

Abstract Details

Purpose:

To prospectively evaluate outcomes of topography-guided LASIK in the treatment of myopia and compound myopic astigmatism.  Outcome measures include high contrast snellen acuity, low contrast snellen acuity (5 and 25%), safety, predictability, efficacy, higher order aberration analysis, changes in epithelial thickness maps using anterior segment OCT and self-reported quality of vision.

Methods:

Twenty eyes of ten consecutive myopic patients underwent topography-guided LASIK (Contoura) using the Alcon Allegretto excimer laser system.  The mean pre-operative spherical equivalent refraction was -2.81 +/- 1.69 diopters (range -0.75 to -6.50 diopters).  The mean cylinder was +0.61 +/- 0.83 diopters (range 0.00 to + 3.00 diopters).  Epithelial thickness maps were measured using the OptoVue anterior segment ocular coherence tomography system.  Epithelial thickness maps were analyzed centrally, superiorly, inferiorly, nasally and temporally preoperatively and at post-operative months one and three.

Results:

At three-months, mean spherical equivalent was +0.09 +/- 0.35 diopters (range -0.25 to +0.75 D).  At three months, mean cylinder was +0.20 +/- 0.49 diopters (range -0.25 to +1.13 D).   At three months, 100 and 75% of eyes had an UDVA of 20/20 and 20/16, respectively.  No eyes lost more than one line of CDVA.  Mean central epithelial thickness was 54.88 +/- 3.93 um pre-op and 54.58 +/- 3.90 um and 55.00 +/- 2.98 um at one and three months, respectively.  There was non-significant thinning of epithelial thickness maps superiorly, inferiorly, temporally and nasally at post-op months one and three.

Conclusions:

opography-guided LASIK using the Allegretto excimer laser yielded excellent safety, efficacy and predictability in myopic eyes.  There were minor changes in epithelial thickness maps from pre-op to post-op months 1 and 3.   We plan to update the cohort to include data from 60 eyes of 30 subjects.
