Purpose:
To analyze the clinical outcomes of 10 consecutive complex patients undergoing reconstructive iris surgery with the artificial iris implantation using a four floating suture technique.
Setting:
Eye Hospital, University Medical Centre Ljubljana, Slovenia
Methods:
We conducted a retrospective case series study evaluating the history charts of 10 consecutive patients (9 adults , 1 child) undergoing a secondary (trauma) or primary (aniridia) reconstructive iris surgery; with the implantation of an artificial iris (HumanOptics), which was combined with a primary PKP in 3 patients and complicated cataract surgery in 7 patients. Preoperative clinical evaluation included BCVA and IOP. The outcome after surgery was: BCVA, IOP, IOL/artificial iris centration status and aesthetic results. The postoperative absence of inflammation was monitored and macular thickness was measured using OCT technology.
Results:
In the current retrospective study 7 males,2 females and 1 child were followed for at least one year (longest period 48 months);BCVA pre-op was 0.4 (logMAR 0.8), with an IOP of 16 mmHg.Mean postoperative BCVA was 0.5 (logMAR 0.62) thus vision remained stable in most reported cases and mean IOP of 14 mmHg under topical antiglaucoma medication.In the current follow up period there were no secondary glaucoma complications nor postoperative inflammation. Iris/IOL decentration occurred only in 1 patient (12.5%).Postoperative UBM showed appropriate artificial iris/IOL position with a good cosmetic outcome in most cases and OCT imaging showed no macular pathology.
Conclusions:
The artificial iris implantation using four floating suture technique is a safe procedure with good functional and aesthetic outcomes. Close, long-term monitoring is essential for the success of this surgery.
