Unilateral post-LASIK ectasia despite normal tomography

Abstract Details

Purpose:

To report an unusual case of unilateral post LASIK ectasia despite a completely normal pre-operative corneal tomography.

Setting:

An unusual case report on post LASIK ectasia diagnosed in a young patient with normal tomography.

Methods:

A 24 year-old male patient presented to us for refractive correction. He had a stable refraction for the past 12 years. His best corrected visual acuity in right eye was 20/20 with -0.75DS/-1.25DC and in the left eye 6/6 with 1.25DS/-0.75DC. All pre-refractive surgery tests, including tomography were normal for both eyes with a pachymetry of 539 microns in right eye and 548 microns in left eye. Randelman Risk scoring indicated the patient to be low risk of post LASIK ectasia. He underwent uneventful LASIK with an intended flap of 110 microns with microkeratome.

Results:

His UCVA was 20/20 at 1 day, 1 week and 3 weeks postoperative. At 6 months postoperative, the UCVA in the RE dropped to 20/40 with a refraction 0.25DS/-1DC. Topography showed a mild steepening in the infero-nasal quadrant, which progressively increased over the next year. A diagnosis of Unilateral Post LASIK ectasia was made and Corneal Collagen crosslinking (CXL) was done for the right eye to stop the progression of ectasia. At one year post CXL, there is stable topography with BCVA of 20/20 in both eyes.

Conclusions:

Post lasik ectasia may develop despite normal tomography.
