Consideration of the duration of the intraocular lens (IOL) dislocation for each risk factor after the initial cataract surgery

Abstract Details

Purpose:

In this study, we aimed to verify the duration until the onset of postoperative intraocular lens (IOL) dislocation (including subluxation) for each risk factor for in-the-bag and out-of-the-bag fixation of IOL during the initial cataract surgery.

Setting:

Retrospective study.

Methods:

We verified the time until the onset of IOL dislocation from the initial surgery for each risk factor for 276 eyes of patients who had undergone initial surgery . A total of 14 risk factors for IOL dislocation were identified: 11 risk factors associated with underlying diseases (history of trauma, pseudoexfoliation syndrome, atopic dermatitis, retinitis pigmentosa, history of glaucoma attack, glaucoma, short axial length of the eye, long axial length of the eye, history of uveitis, postoperative retinal detachment and 3 associated with surgical findings (rupture or vulnerability of the zonule of Zinn, IOL fixation position, and posterior capsule rupture).

Results:

A total of 276 eyes were investigated. The median duration until the onset of dislocation from the initial surgery was 118 months. Targeting only the cases of in-the-bag fixation of IOL during the initial surgery, risk factors occurring within each time point (12, 60,120 months) prior to dislocation were examined using logistic regression analysis. As risk factors for the onset of dislocation within 12 months, pseudoexfoliation syndrome and rupture or vulnerability of the zonule of Zinn were higher risk. Within 60 and 120 months, rupture or vulnerability of the zonule of Zinn was higher risk.

Conclusions:

The IOL fixation method during the initial cataract surgery should be considered based on the degree of risk of the aforementioned risk factors.
