Toric intraocular lenses in the real world: rotational performance and visual outcome – does the astigmatism axis matter?

Purpose:
To evaluate the rotational stability, its influencing factors and the visual outcome of an aspheric one-piece hydrophobic acrylic toric intraocular lens (IOL) in a public hospital with high astigmatism pacients.
Setting:
Department of Ophthalmology Hospital de l'Esperança - Parc de Salut Mar, Barcelona, Spain.
Methods:
Retrospective study performed in a tertiary care center in Spain. Thirty eyes with visually significant cataract and regular corneal astigmatism greater than 2 diopters (D) were included. They underwent standard phacoemulsification and were implanted with a toric IOL.
Patients were called for follow up on day 1, 1 week, 1 month, and 3 months, postoperatively.
The axis of the toric IOL, was measured by slit lamp. The analysis was performed in three separated groups by the astigmatism axis (Group A: 180º+/-20º), (Group B: 90º+/-20º) and (Group C: between 20º and 70º; and between 110º and 160º).
Results:
The mean preoperative refractive astigmatism was 3,39 (2,39) D and the mean postoperative at three months was 0,65 (0,49) D (p<0,05).
The preoperative visual acuity was 0,46 (0,25) logMAR and postoperative 0,24 (0,18) logMAR (p<0,001).
At day 1 misalignment was 3,39º (5,14º) (p=0.003); between day 1 and 1 week 1,03º (4,57º), between 1 week and 1 month 0,5º (1,9º) and between 1 and 3 months 0,3º(1,1º)
The mean rotational misalignment at three months was for Group A 3,29 (5,53) , Group B 7,92 (7,9) and Group C 2,78 (4,24) (p=0,14).
Conclusions:
The visual acuity and the refractive astigmatism were significantly corrected in all groups.
The 57% of toric IOL tended to rotate postoperatively and 13% more than 10 degrees which required surgical rotation.The rotation was significantly increased within the first day operation compared with later time points (p=0,003).
Although it seems affected by the astigmatism axis, there was not statistically significant difference. Further prospective studies are needed in order to elucidate this results.
