Abstract Details

Purpose:

To compare and examine the histology of the incisal surface of the lens capsule by 4 types of anterior capsulotomy methods.

Setting:

This study used 15 porcine eyes in vitro.

Methods:

Anterior capsulotomy were performed to collect tissues from 15 pig eyes by 4 methods in which anterior capsular forceps were used on 2 eyes, femtosecond laser (LenSx, Alcon) on 5 eyes, precision pulse capsulotomy (Zepto, Mynosys)on 4 eyes, and laser anterior capsulotomy (CapsuLaser, EXCEL-LENS) on 4 eyes. 15 capsular bags after the anterior capsule incision were fixed with 2.5% glutaraldehyde. Fixed lens capsules were dehydrated and dried, platinum coated, and observation of the cut end surfaces were performed using a scanning electron microscope (FlexSEM 1000, Hitachi).

Results:

It was confirmed that collagen fibers traveled at the ablation stump surface when anterior capsular forceps were used. In all femtosecond laser cases ablation stump surfaces were irregular, and partial damages were recognized except for the ablation stump surface. In all Zepto cases, a part of the ablation stump surface was irregular, a part changed to a smooth surface, and the ablation stump surfaces were warped by the influence of the modification and suction. In all CapsuLaser cases, all the ablation stump surfaces were uniformly irregular, and were distorted due to denaturation.

Conclusions:

Different tissue images were observed for each method of anterior capsulotomy. Whether the differences in histopathology are clinically significant or whether the human eye exhibits a similar histology should be further studied.
