Pachymetric predictability factor for early graft detachment in primary Descemet's membrane endothelial keratoplasty

Abstract Details

Purpose:

The evaluation of central pachymetric variation between the day prior to the the surgery and the day after is a simple predictive factor for early graft detachment after Descemet Membrane Endothelial Keratoplasty.

Setting:

Prospective and Monocentric study at the Rothschild Foundation in Paris from september 2019 to march 2019 including forty seven eyes of forty seven patients that had Descemet Membrane Endothelial Keratoplasty asociated or not with phakoemulsification.

Methods:

Central pachymetry was measured by optical coherence tomography (OCT) swept source (RTVue OCT de Optovue, Inc) the day prior to the the surgery and the day after (D-1 and D+1). All patients were transplanted with 8mm diameter endothelial grafts, and all eyes were filled with air at the end of surgery. All patients respected a strict dorsal decubitus for twenty four hours and were examined with a corneal OCT the first ten days after surgery and one month after. They all received a treatment of four drops a day of dexamethasone and lubricants.

Results:

Among the forty seven eyes, 13 (27,6%) showed an early graft detachment during the first 10 days after surgery with a mean central pachymetric variation between  D-1 et D+1 of +21% (from +4% to +60%, standard deviation of 21%). Among eyes without graft detachment (72,4%), the mean central pachymetric variation was -1% (from -35% to +22%, standard deviation of 12%) p-Value of 0,002.

Conclusions:

The central pachymetric variation measured by swept source OCT between the day prior to the the surgery and the day after is a good predictive factor when increasing, for early graft detachment after Descemet Membrane Endothelial Keratoplasty.
