Purpose:
The implantation of the IC-8 small aperture IOL for patients with irregular corneal astigmatism undergoing lens exchange surgery might improve visual acuity and visual quality due to a pinhole effect. Even more these patients might benefit from an enhanced depth of focus.

University Eye Hospital Munich, Germany
Methods:
This prospective, non-randomised, single-centre, interventional case series enrolled patients with irregular corneal astigmatism (mild or moderate non-progressive keratoconus, pellucid marginal degeneration, corneal scars, ectasia after radial keratotomy and keratoplasty) undergoing lens exchange surgery. Further inclusion criteria were CDVA ≤ 0.7 Snellen, age above 45 years and intolerance in wearing rigid contact lenses. Preoperative and three months postoperative UDVA, UIVA, UNVA, CDVA, CIVA, CNVA, macular thickness change (OCT) and rate of early posterior capsular opacification were evaluated. Defocus curve was also assessed. Quality of life questionnaire (NEI-RQL-42TM) was used to evaluate the subjective improvement.
Results:
Surgery was uneventful in all cases. Visual acuity showed an improvement in uncorrected distance, intermediate and near visual acuity of 0.28 LogMAR, 0.23 LogMAR and 0.16 LogMAR, respectively. Defocus curve showed an enhancement of depth of focus in all patients, limited to the amount of irregular corneal astigmatism. Posterior capsular opacification requiring treatment within three months was observed in two cases (12%). Significant macular edema was not observed in any case. The analysis of the quality of life questionnaire on subjective complaints such as dysphotopsia and photophobia showed a significant improvement in the perception of optical phenomena.
Conclusions:
The implantation of the Small-Aperture IC-8 IOL during lens exchange surgery in patients with irregular corneal astigmatism is a safe and effective surgical procedure with significant restoration of visual acuity and reduced perceived optical phenomena.
