Purpose:
To compare the optical performance of a new monofocal intraocular lens (IOL) designed to extend depth of focus (Tecnis® Eyhance, model ICB00) with a standard monofocal IOL (Tecnis® 1-piece, model ZCB00) and a diffraction-based extended depth of focus (EDOF) IOL (Tecnis® Symfony, model ZXR00).

Methods:
Assessment of the optical quality of the three IOLs was made in-vitro using an optical test bench with a model eye that is consistent with the International Organization for Standardization recommendations (ISO 11979-143 2:2014.2). The measurements were performed with two pupil sizes (3.0 and 4.5 mm). The optical quality was analyzed using several image quality metrics, such as the Modulation Transfer Function (MTF) and the area under the MTF (MTFa), obtained through focus and at the best image planes. Additionally, halo formation and its dependence on pupil size were assessed at the distance focus of the lenses.
Results:
At the focus for distance vision, with a 3.0mm pupil, the MTF (at 50cycles/mm) of the Eyhance IOL was 0.62±0.03, slightly lower than the standard monofocal ZCB00 (0.78±0.02) but considerably better than the EDOF Symfony (0.27±0.02). Conversely, at an intermediate distance (+1.70D) Symfony reached the highest MTF (0.49±0.02) and therefore, best optical quality, whereas Eyhance and ZCB00 showed much lower scores (0.08±0.01 and 0.007±0.001, respectively). All lenses had worse results with a 4.5mm pupil. The through-focus MTFa curves of Eyhance and ZCB00 were quite similar. Eyhance and ZCB00 formed similar small halos.
Conclusions:
The optical quality of the Tecnis® Eyhance focus for distance vision is slightly lower than the standard monofocal ZCB00 IOL’s, but remarkably better that EDOF (Symfony) IOL’s. For the three IOLs, such quality worsens with a larger pupil. The analysis of the through-focus MTFa curves did not show clear evidence that the Eyhance IOL provided focus extension as compared to the standard ZCB00 lens. Finally, the halos formed by Eyhance and ZCB00 were similar and, in all cases, less significant than those formed by the EDOF Symfony IOL.
