Purpose:
To quantify the difference between predicted and actual refractive outcome of eyes that underwent phacoemulsification combined with iStent.
Setting:
Western Eye Hospital, Imperial College Healthcare NHS Trust, London, UK
Methods:
Retrospective case-series of consecutive glaucoma patients undergoing phacoemulsification-iStent in a tertiary referral centre. Post-operative refractive outcomes were obtained by subjective refraction or autorefraction. The difference between the predicted spherical equivalent and the actual post-operative spherical equivalent was calculated for each patient. This was then compared to national guidelines.
Results:
52 eyes undergoing phacoemulsification with iStent were included. 87% of eyes (n=46) achieved a post-operative spherical equivalent within +/- 1 dioptre of the target refraction. This exceeds the national standard of 85% for cataract surgery. Mean (±SD) pre-operative visual acuity was 0.72 ± 0.74 LogMAR. Mean (±SD) post-operative visual acuity was 0.21 ± 0.40 LogMAR (p < 0.001).
Conclusions:
This study suggests that iStent insertion during cataract surgery does not significantly alter effective lens position. Therefore, no modification of biometry formulae is required when selecting lens power. In glaucoma patients with co-existing cataract, Phacoemulsification-iStent should be considered as a safe surgical treatment option in terms of refractive outcome and visual acuity.
