Purpose:
Clinical evaluation of the visual and reading performances after a combined implantation of a small aperture intraocular lens (IC-8 [Acufocus]) in one eye and a segmental refractive bifocal IOL (Lentis Mplus LS-313 MF20 [Oculentis] with a near addition of +2.0 D) in the fellow eye.

Methods:
In this ongoing prospective study, 26 eyes of 13 patients have been treated so far. Follow-up examinations included: uncorrected (UDVA) and best-corrected distance visual acuities (CDVA), uncorrected (UIVA) and distance-corrected intermediate visual acuities (DCIVA), and uncorrected (UNVA) and best-corrected near visual acuities (BCNVA). Furthermore, distance-corrected reading acuity at 40 cm for near, 80 cm for intermediate distance, and at the patients’ preferred near and intermediate distances were evaluated using the Salzburg Reading Desk (SRD). Photic phenomena were also evaluated using a computer-based Halo and Glare simulator and the defocus curves were assessed between -5.0 D and +2.0 D.
Results:
Mean UDVA was -0.04 logMAR (0.12 to -0.26 logMAR), UNVA was 0.11 logMAR (0.20 to
-0.08 logMAR), and UIVA was 0.00 logMAR (0.14 to -0.14 logMAR). The improved performance at intermediate distances was confirmed by the SRD. The reading acuity was 0.11 logMAR at a preferred intermediate distance of 78.03 cm in contrast to a reading acuity of 0.21 logMAR at a preferred near distance of 40.85 cm. Numeric value for Halo size was 35.25 (0 to 68), for Halo intensity 36.33 (0 to 79), for Glare size 14.63 (0 to 60), and for Glare intensity 28.63 (0 to 50).
Conclusions:
The new concept of a combined implantation of a small aperture IOL and a segmental refractive bifocal IOL provided good results at far and intermediate distances as well as functional results at near distance with low levels of photic phenomena. Patients reported a high rate of satisfaction and spectacle independence at various distances.
