Time-averaged long-term outcomes after small-incision lenticule extraction for myopia correction: comparison with femtosecond laser-assisted LASIK

Abstract Details

Purpose:

There are very few publications reporting long-term clinical outcomes after ReLEx SMILE. The aim of this retrospective analysis is to apply a novel statistical approach in order to investigate whether 5 years after myopia correction visual outcomes for SMILE are as safe, predictable and efficient as those after topography-guided Femto-LASIK.

Setting:

All ReLEx SMILE (VisuMax, CZM) and Femto-LASIK (Mel 80, CZM) surgeries were performed by one surgeon (BY) at the Breyer-Kaymak-Klabe Eye Surgery & Premium Eyes in Duesseldorf, Germany, which is part of the International Vision Correcton Research Center (IVCRC.net).

Methods:

The results of 1190 (371) consecutive myopic eyes treated with ReLEx SMILE (Femto-LASIK) are evaluated retrospectively. Follow up was performed between 1 day and 5 years after treatment. For clinical evaluation we calculated time-averaged frequency-weighted means (TAFWM) for visual acuity at far, subjective refraction, safety, efficacy, predictability, and residual astigmatism.

Results:

TAFWM for safety, efficacy, and predictability were (-0.01+-0.07)logMAR and (0.00+-0.07)logMAR, (0.03+-0.10)logMAR and (0.04+-0.10)logMAR, and (-0.01+-0.37)D and (-0.15+-0.38)D, respectively. UDVA (only taken for subgroups with emmetropic target refraction) and CDVA values were (0.01+-0.10)logMAR and (0.03+-0.10)logMAR and (-0.03+-0.07)logMAR and (0.00+-0.07)logMAR, respectively. Residual astigmatism was (-0.29+-0.30)D and (-0.31+-0.31)D. Fewer complains of dry eye were observed in the ReLEx SMILE group compared to the Femto-LASIK group.

Conclusions:

Compared to mean values at distinct time intervals, calculating time-averaged frequency-weighted means reduces the impact of measurement inaccuracies on observed variances. Five years after surgery, ReLEx SMILE and Femto-LASIK show equally good results. Differences in TAFWM were either statistically not significant or clinically not relevant (approx. at most 1 letter).
