New monofocal IOLs and the challenge of the intermediate distance

Abstract Details

Purpose:

To assess visual outcomes, contrast sensitivity, defocus curve, optical quality and spectacle independence in patients implanted with a new monofocal intraocular lens (IOL) designed to enhance intermediate vision (Tecnis Eyhance ICB00 IOL), in comparison with the Tecnis ZCB00 IOL.

Eye Clinic, University of Florence, Italy

Methods:

This prospective, nonrandomized, comparative study included 24 patients scheduled to undergo cataract surgery with bilateral implantation of two monofocal IOLs: Tecnis ZCB00 IOL (24 eyes) or Tecnis Eyhance ICB00 IOL (24 eyes) (Abbott Medical Optics Inc.). Monocular and binocular uncorrected (UDVA) and corrected (CDVA) distance visual acuities, uncorrected and distance-corrected and corrected intermediate (UIVA, DCIVA, CIVA) and near (UNVA; DCNVA; CNVA) visual acuities, photopic contrast sensitivity and defocus curve were measured at one postoperative month. Objective scatter index (OSI), Strehl Ratio and modulation transfer function (MTFcut-off) were evaluated by OQAS (Visiometrics, Spain). Quality-of-vision and spectacle-dependence questionnaires were also administered.

Results:

Monocular and binocular distance and near visual acuities were similar between groups. Monocular and binocular UIVA and DCIVA were significantly higher in the ICB00 group. Photopic contrast sensitivity was similar for any spatial frequency (p>0.05 for all comparisons). Values of OSI, MTF cut-off and the Strehl ratio were similar between groups (p= 0.239, p= 0.759 and p=0.144, respectively). No statistically significant difference between the two IOLs was detected regarding glare and halo perception, evaluated by the NEI-RQL 42 questionnaire “glare” subscale (p=0.30). Moreover, the ICB00 IOL resulted in better spectacle independence for the intermediate distance than the ZCB00 IOL.

Conclusions:

In the current study, both the evaluated IOLs showed very good UDVA, with significantly higher visual results at the intermediate distance (66 cm) in the Eyhance ICB00 group. According to administered questionnaires, patients implanted with Eyhance IOLs reported to be independent from correction for intermediate visual tasks, with a low incidence of unwanted photic phenomena. The optical quality, measured using the OQAS system, was similar in both groups, as well as contrast sensitivity evaluated by sinusoidal gratings method.
