Abstract Details

Purpose:

The Clareon® IOL is an advanced monofocal IOL made from a new hydrophobic acrylic material. Chromatic properties are highly associated to intrinsic material properties defined by the Abbe number. We investigated the longitudinal chromatic aberration (LCA) in patients implanted with the Clareon IOL, both psychophysically and objectively. Additionally, we explored the interactions between chromatic and monochromatic aberrations

Setting:

Methods:

Ten patients (age: 68.4±3.2; Spherical Equivalent: 0.34±0.6D) were implanted with Clareon IOLs (IOL power: 19-23D) following standard phacoemulsification. We used a polychromatic adaptive-optics system, provided with a supercontiuum laser, Hartmann-Shack aberrometer, pupil monitoring and fixation. Wave aberrations were obtained at 5 wavelengths (480-700nm). Subjective settings of best-focus were performed while viewing a DLP-projected target, illuminated with the same wavelengths. Objective and subjective LCA were obtained from the measured chromatic difference of focus. MTFs for green at best-focus and blue defocused by the subjective LCA were estimated to account for the relative degradation imposed by LCA (for Clareon, Acrysof and Tecnis)

Results:

Average subjective LCA (480-700 nm) was 1.220.09D, and objective LCA was 0.92±0.21D. Visual Strehl (VS) ranged from 0.84 to 0.3. MTF in blue decreases significantly with respect to the MTF in green, with a MTF green/blue ratio ranging from 3.82 to 1.38. There is a statistically significant positive correlation between VS and MTF green/blue ratio (p<0.01, r=0.806). The impact of LCA on the MTF is comparable in Clareon and Tecnis material, and significantly lower (factor of 1.3 in the MTF green/blue ratio) in Acrysof material.

Conclusions:

As consistently found in previous studies the objective LCA is lower than the subjective LCA, likely due to the spectral reflectivity of the retina. The LCA for Clareon is lower than for the Acrysof material and similar to Tecnis. The impact of the LCA on image degradation is highly dependent on the presence of monochromatic aberration and interactive effects, being relative larger in the least aberrated eyes.
