One-year refractive outcomes after enhancement by optic exchange among eyes that received a multicomponent intraocular lens

Abstract Details

Purpose:

Accurate and stable refractive outcomes are essential for the long-term success of premium intraocular lenses (IOL). Multicomponent IOL’s (MCIOL) allow refinements after primary surgery by enabling surgical exchange of one of its refractive components. We report the one-year visual and refractive outcomes after performing optic exchange (also known as enhancement surgery) among eyes that underwent cataract surgery and received an MCIOL.

Setting:

All surgeries and follow up visits were performed at the Ambulatory Surgical Center, Peregrine Eye and Laser Institute, Makati, Philippines

Methods:

Three months after cataract surgery with in-the-bag MCIOL implantation, eyes with residual manifest refraction spherical equivalent (MRSE) more than 0.75 diopters(D), underwent optic exchange (enhancement) procedure. The original front optic was detached from the base optic and removed through the original incision using IOL forceps. A new front lens with corrected refractive power based on the 3-month MRSE, was injected into the anterior chamber and attached to the base lens. Main outcome measures: change in post-enhancement uncorrected distance visual acuity (UDVA) in logMAR and change in MRSE from the preoperative to 3-month; and, from the 3-month to 1-year visit.

Results:

Sixteen eyes underwent optic exchange/enhancement and completed their 1-year follow up visits. The mean (SD) pre-enhancement UDVA was 0.2 (0.07) improving to 0.0 (0.1) at the 3-month visit (P=0.008) and remaining stable at 0.00 (0.1) on the 1-year visit (P=0.12). After enhancement, the mean pre-enhancement MRSE was +1.3 (1.14) diopters and following enhancement, significantly decreased to +0.04 (0.38) (P<0.001) at the 3-month visit (P<0.001) and remained stable at +0.1 (0.44) D on the 1-year visit (P=0.95). The enhancement procedure was simple and completed within 5-minutes or less. No eye developed significant adverse events within the 1-year follow-up period.

Conclusions:

MCIOL's provided a safe and predictable method of improving refractive and visual outcomes after cataract surgery. Long-term clinical results appear stable. MCIOL appears to be a reliable method for correcting post-operative refractive surprise.
