Abstract Details

Purpose:

To report a case of bilateral total iris atrophy and refractory glaucoma in a patient few years following cosmetic artificial iris implants

Setting:

Amman/Jordan

Methods:

A thirty eight year old Jordanian woman presented with a 2-week history of progressive visual loss in the right eye. Her past ophthalmic history was significant for cosmetic anterior chamber iris implant bilaterally (OU) (BrightOcular®) 3 years prior to consulting us. Her intraocular pressure (IOP) was reported to be 60 mmHg OD and 38 mmHg OS for which she had undergone an unsuccessful attempt to remove the prosthesis OD in addition to an Ahmed glaucoma valve (AGV) implanted in the same eye.

Results:

On presentation, visual acuity as 0.1 OD and 0.8 OS. IOP was 6 mm Hg OD and 17 mmHg OS. Right eye exam showed moderate diffuse corneal edema. Light-blue iris prosthesis were seen OU with a small vertical inferior crack OD. Optic nerve head exam showed a cup-to-disc ratio of 0.85 OD and 0.5 OS. Specular microscopy showed an endothelial cell count of 732 OS but could not be measured OD. Central corneal thickness was 811 OD and 638 OS. Bilateral removal of the iris implants was done and to our surprise total iris atrophy was found in both eyes.

Conclusions:

This case adds to the long-term sight-threatening complications of cosmetic iris implants. We believe this is the first case report of complete circumferential iris atrophy following artificial iris implantation.
