Abstract Details

Purpose:

To evaluate accommodative changes and visual outcomes in presbyopic eyes after Laser Scleral Microporation (LSM).

Setting:

Subjects were enrolled in an early feasibility study to evaluate the safety and efficacy of the Laser Scleral Microporation (LSM) to restore Extended Range of Focus for presbyopes.

Methods:

This pivotal clinical study evaluated the LSM procedure early results.12eyes of 6 subjects over 40 years of age demonstrating loss of accommodative function, good uncorrected distant vision and otherwise healthy eyes were evaluated. A 2.94um Er:YAG novel scanning laser was used creating a 5x5 microporation matrix in 4 quadrants on the sclera(LSM) to improve biomechanical efficiency of the ciliary muscles in 5 critical zones of physiological function. Primary study targets were improvement in UIVA/DCIVA (60 cm) and UNVA/DCNVA (40 cm) and UDVA/CDVA. using EDTRS logMAR chart. IOP was also assessed using a tonometer .Ray-tracing aberrometer was used to measure EROF.

Results:

LSM provided improvement in both accommodative ability and near visual acuity. EROF improved from 1.31D to approximately 1.60D on average. Binocular uncorrected visual acuity at near (40cm; UNVA) improved from +0.60 ± 0.14 (logMAR) to +0.14 ± 0.17 (logMAR) (p=0.00010). No complications were reported. There was no statistically significant loss in distance vision in these patients. No complications were reported.

Conclusions:

LSM performed using the VisioLite Er:Yag laser appears to be a safe and effective procedure for restoring range of visual performance for near and intermediate visual tasks without compromising UDVA/CDVA or binocularity. LSM appears to improve EROF by restoring some true accommodation as well as supplemental pseudoaccommodation improving quality vision in presbyopic eyes.
