Clear lens exchange with multifocal IOL improves health-related quality of life: one-year prospective follow-up study

Abstract Details

Purpose:

Multifocal IOLs are acknowledged to treat presbyopia, well before cataract is present. This study measures health-related quality of life (HRQoL) after clear lens exchange with multifocal IOLs. Also, visual function is studied.

Setting:

This ethically approved prospective multicenter study consisted of 132 subjects. Bilateral CLE was done in the clinics of Coronaria Medilaser, Finland. All the subjects completed 6-month and one-year follow-ups.

Methods:

HRQoL was measured by the 15D instrument, (Sintonen H, www.15d-instrument.net). Visual function was measured with VF-14 questionnaire. Multifocal IOLs were Abbott Tecnis +3.25 bifocal, Abbott Tecnis Symfony EDOF, Alcon PanOptix trifocal, PhysIOL FineVision trifocal, and Zeiss AT LISA trifocal. Toric versions were used when needed. Baseline BCVA was compared to UCVA 6 and 12 months after surgery. Study subjects mean age was 57 years, 57% females. Spherical equivalent refraction range was from -8.9D to +5.0D, and mean near add +2.1D.

Results:

The mean overall 15D score increased from 0.938 to 0.954 at six months (p < 0.0001), and to 0.948 at 1-year (p = 0.02, compared to baseline). The mean “seeing” dimension score of the 15D improved significantly from 0.870 to 0.962 at 6 months (p < 0.0001), and to 0.977 at 1-year (p < 0.0001). VF14 score increased from 85.33 to 96.45 at six months (p < 0.0001), and to 96.59 at 1-year (p < 0.0001). Baseline BCDVA right/left was -0.07/-0.08 logMAR. Binocular UCDVA at 6/12 months was -0.05/-0.04 logMAR. Binocular baseline BCNVA compared UCNVA 6/12 months did not change.

Conclusions:

CLE with multifocal IOLs improves significantly health related quality of life in 1-year follow-up. The improvement at 6 months was also clinically important (Alanne at al 2014). CLE also improves visual functions compared to eye glasses. Multifocal IOLs demonstrate good near visual acuity which is comparable to spectacles. UCDVA decreases compared to spectacles, but remains in well acceptable level. Use of eyeglasses was minimal after surgery.
