Abstract Details

Purpose:

To estimate the prevalence of continued long term corneal flattening after accelerated cross-linking for progressive keratoconus

Setting:

Moorfields Eye Hospital, London

Methods:

Prospective, uncontrolled case series of eyes with progressive keratoconus, defined by more than 1.5 dioptre (D) increase in Kmax. Eyes with a minimum stromal thickness of less than 375 µm were excluded. Riboflavin 0.1% soak duration was 10 minutes followed by UVA exposure at 30 mW/cm2 for 4 minutes (total energy 7.2 mJ/cm2). The primary outcome measure was a sustained, progressive decrease in Kmax over 5 years folllow-up.

Results:

55 eyes met the inclusion criteria for 5 years follow-up of which 1 eye exhibited sustained, progressive flattening. Baseline Kmax was 55.2 D, reducing to 54.1 D (1 year), 52.8 D (2 years) and 50.8 D (5 years). The prevalence is estimated at 1.8%.

Conclusions:

Continued long term flattening following accelerated cross-linking is less common than following Dresden-protocol cross-linking.
