12 years' correction of high astigmatism (>2.5D) with an aspheric ablation profile: results for the AMARIS since 2007

Purpose:

The aim of this retrospective analysis is to evaluate how the outcomes of high-astigmatism evolved during 12 years of treatments using the SCHWIND AMARIS system

Setting:

Muscat Eye Laser Center

Methods:

Treatments since 2007 have been audited, and the subset of treatments with planned cylinder above 2.5D have been selected. Treatments have been segregated by treatment year, and refractive and visual outcomes have been evaluated “year by year” for a constant follow-up of 3-months. A secondary aim has been to identify the treatments with “long follow ups” (>6 years), and from those long term stability of the outcomes has been assessed. Wherever possible, stratifications of the analyses by technique (LASIK vs PRK) have been conducted.

Results:

327eyes of 231 patients with preop astigmatism >2.5D retrospectively evaluated (approximately 28eyes ,20 patients per year). At 3m, 85% achieved 20/20 or better UDVA and 45%eyes achieved 20/16 or better UDVA. 55%eyes were within +/-0.25D of the attempted astigmatic correction, and 85% within +/-0.50D. The mean postoperative SE was -0.12D +/- 0.25 and mean astigmatism, 0.55 +/- 0.26 D. CDVA improved in 40%eyes; 3%eyes lost 1 line of CDVA. The predictability slope for astigmatism was 0.97 and the intercept, -0.1D. A subtle trend for improvement through the years could be detected, no sudden/abrupt changes or isolated peaks in performance.

Conclusions:

From 2007 until now, in 12 years of review, we see a smooth trend for the outcomes to improve through the years, possibly indicative of subtle refinements in techniques and technology reaching a saturation plateau. We have not detected sudden changes indicative for a change in technology, or any isolated peaks up or down in performance indicative of technical instabilities.
