Abstract Details

Purpose:

To carry out a comparative analysis of the long-term results of femtosecond laser-assisted phacoemulsification with standard ultrasound phacoemulsification technique in cataract complicated by weakness of the zonular support of the lens.

Setting:

The work was performed in the Khabarovsk branch of the Federal State Autonomous Institution «S.N. Fedorov National Medical Research Center «MNTK «Eye Microsurgery» of the Ministry of Health of the Russian Federation

Methods:

30 eyes - femtosecond phacoemulsification and 30 eyes - standard ultrasound phacoemulsification. The stability of the IOL central position was assessed in biomicroscopy and computerized topography on the rotary Shaimpflug camera OCULUS PENTACAM HR (Germany). The periods of dynamic observation were to 4.5 years. The IOL position was estimated in frontal and sagittal planes. In frontal IOL displacement the clinical classification of IOL decentering was used (S.N. Fedorov, E.V. Egorova, 1992). IOL inclination in sagittal plane was determined relative to iris plane in degrees (PENTACAM HR). Frequency of examination: 1 day, 2-3 days, 1, 2, 3 and 4.5 years.

Results:

In all 60 eyes, at discharge, the correct IOL intracapsular position was determined. Non-corrected visual acuity (NCVA) averaged 0.58±0.19. In all cases after femtosecond phacoemulsification, for a period of 4.5 years, the IOL centered position was preserved both in the frontal and sagittal planes, the NCVA on average: 0.7±0.16. In the group with manual phacoemulsification, in 4.5 years, the NCVA averaged 0.47±0.15. IOL dislocation in this group occurred in 2 eyes (7%), although it does not give significant difference. More reliable results can be evaluated at more remote periods of observation.

Conclusions:

Thus, femtosecond laser-assisted phacoemulsification in cataract surgery, complicated by weakness of the zonular support of the lens, showed high clinical efficacy, which consisted in the absence of significant intraoperative complications, areactive course of the early postoperative period, the stability of the IOL position throughout all the observation period up to 4.5 years.
