Abstract Details

Purpose:

To investigate whether the rhexis size remains stable during phacoemulsification surgery and after cataractous lens material is removed and the bag is filled with an artificial lens that is smaller in size than the native lens.

Setting:

Department of ophthalmology, Amsterdam University Medical Centers, location AMC, Amsterdam, the Netherlands.

Methods:

Videos of phacoemulsification surgeries where sampled. Samples were taken immediately after the rhexis was completed, after the bag was filled with viscoelastic before implantation of the IOL, and after implantation of the IOL and removal of viscoelastic from the eye. Each point in surgery was sampled 3 times. The samples were compared using software and assessed for 1) perimeter, 2) area size and 3) largest diameter as a proxy for change in diameter size, either shrinkage or stretch of the rhexis.

Results:

Samples were taken from 8 surgical videos. The mean rhexis area at completion of the rhexis was the baseline measurement in each eye. The next 2 measurements where compared to this. The mean rhexis measurements pre-implantation were 1.9, 1.7 and 1.5 % larger respectively for perimeter, area and largest diameter. The mean rhexis measurements at conclusion of the surgery were 6.7, 5.5 and 6.2 % smaller respectively for perimeter, area and largest diameter as compared to pre-implantation. Which was a significant decrease.

Conclusions:

Capsulorhexis size decreases during phaco surgery.
