Abstract Details

Purpose:

To compare the safety and efficacy of the Hyper-CL™ lens versus a standard bandage contact lens (PureVision B&L) for chronic corneal edema.

Setting:

Prospective, multi-center, randomized, crossover study. Four outpatient clinics of tertiary centers: Tel Aviv University Sheba Medical Center Tel Hashomer; Soroka University Medical Center, Ben-Gurion University of the Negev, Beer-Sheva, Israel; Silesian University of Medicine, Katowice, Poland; Medical University, Lublin, Poland.

Methods:

Chronic corneal edema patients were randomized to either Hyper-CL™ lens or standard soft bandage contact lens. Both groups also received 5% sodium chloride daily. After a 7-day treatment period there was a 7-day washout period after which the groups were crossed over. Patients were evaluated at days 0 (baseline), 7 (following first treatment allocation), 14 (following washout) and 21 (following second treatment allocation). The primary outcomes were central corneal thickness (CCT) and best corrected visual acuity (BCVA) and success was defined as a reduction >8% in CCT and/or >1 ETDRS line improvement in BCVA.

Results:

In total, 49 patients were enrolled. There was a significantly greater proportion of success in the Hyper-CL group (58.7% versus 39.1%, p=0.02). There was significantly greater BCVA improvement rate >1 line (47.8% versus 34.8%, p=0.04), >2 lines (43.5% versus 28.3%, p=0.03) and >3 lines (30.4% versus 17.4%, p=0.04). The mean change in BCVA lines was significantly greater for the Hyper-CL lens (3.4±6.7 versus 0.9±2.3, p=0.02). There was a greater proportion of patients with a >8% reduction in CCT in the Hyper-CL group (21.7% versus 8.9%, p=0.05). Both lenses were equally safe and tolerable.

Conclusions:

The Hyper-CL™ lens was associated with a higher chance for significant visual acuity improvement and reduction in central cornea thickness when compared to a standard bandage contact lens combined with 5% sodium chloride.
