Corneal optical densitometry following femtosecond laser-assisted DSEK vs microkeratome-assisted DSAEK: a comparative study

Abstract Details

Purpose:

To compare the values of corneal densitometry (CD) as measured by the Scheimpflug-based device (Pentacam, Oculus) following femtosecond laser-assisted DSEK and microkeratome-assisted DSAEK

Setting:

S. Fyodorov Eye Microsurgery State Institution,Moscow

Methods:

37 eyes of 37 patients with Fuchs endothelial corneal dystrophy and cataract were enrolled. 20 patients underwent cataract surgery combined with Femto-DSEK (group A) and 17 patients underwent cataract surgery combined with DSAEK (Group B). Outcome measures included Scheimpflug-based imaging of densitometry data in 4 corneal layers (anterior layer [AL], central [CL], posterior [PL] and total [TL]) in 2 zones: central annular zone (0–2 mm) and mid-peripheral zone (2-6 mm). Other analyzed parameters were including central corneal thickness (CCT), best spectacle-corrected visual acuity (BSCVA) and endothelial cell density (ECD). All patients were assessed 12 months after the initial surgery.

Results:

Corneal densitometry decreased in all 4 layers in both groups over 1 year after the surgery. Postoperatively AL, CL, TL values did not differ significantly between all groups (p>0.05). Femto-DSEK showed higher PL values compared with DSAEK at 1 year follow up. (p=0.037) Postoperative BSCVA differ significantly between the groups (median 0.5 (0.32;0.5) for group A and 0.3 (0.20;0.4) for group B, p=0.012. ECD loss at 1 year in the group A was higher than group B (66.3±8% and 58.9±9.6%, respectively), (p=0.039). CCT didn’t differ significantly between both groups.(p=0.183)

Conclusions:

Corneal densitometry showed a significant reduction in the long term period in both study groups. After one year, CD values (AL,CL,TL) were comparable between both groups throughout the follow up period. PL values were lower in group B than group A. Moreover, BSCVA and ECD were also significantly better after DSAEK.
