Three-year outcomes of mixed astigmatism correction with single-step transepithelial PRK

Abstract Details

Purpose:

Transepithelial photorefractive keratectomy, where both the epithelium and stroma are removed in a single-step (transPRK), is a relatively new procedure of laser refractive error correction. The purpose of this study is to evaluate refractive and visual outcomes of transPRK in the treatment of mixed astigmatism with the use of aberration-neutral profile and large ablation zone.

Methods:

This retrospective, observational case series comprised 47 eyes of 27 patients who underwent transPRK to correct mixed astigmatism and completed the 3-year follow-up. Mean spherical manifest refraction was 1.27±0.65 Dsph (0.5 to 3.0 Dsph) and mean manifest astigmatism was -3.88±0.43 Dcyl (-3.25 to -4.5 Dcyl). All procedures were performed with Amaris 750S excimer laser (Schwind eye-tech-solutions GmbH, Kleinostheim, Germany) using aberration-neutral profile. Eyes treated with optical zone of 7.5 mm or more were exclusively included in the study. Refractive results, predictability, safety, and efficacy were evaluated in a 3-year follow-up.

Results:

Three years after the surgery mean MRSE was -0.09±0.71Dsph, mean cylinder was -0.48±0.62Dcyl. Attempted spherical equivalent correction within ±0.50 Dsph was achieved in 80.8% of eyes, and in 68.1 % of cylindrical correction. Preoperative CDVA was 20/20 or better in 85.1% of eyes and postoperative UDVA was 20/20 or better in 61.7% Three years post-operatively, 31,9% of eyes lost one line of CDVA and 27.7% gained a line. In 12,8% of the eyes haze of low intensity was observed at the periphery.

Conclusions:

Mixed astigmatism correction with large ablation zone transPRK provides good results in terms of efficacy, safety, predictability, and visual outcomes in a 3-year follow up.
