Myopic LASIK and SMILE (L&S) retreatment with topography-guided PRK (tPRK): long-term refractive and corneal imaging evaluation

Abstract Details

Purpose:

To evaluate the safety and efficacy of tPRK retreatment based on TMR after myopic LASIK and Smile.

Setting:

LaserVision gr Clinical and Research Eye Institute, Athens, Greece

Methods:

58 eyes of 34 L&S patients had tPRK with topography-modified refraction (TMR), 27-162 months following the initial procedure. Peri-operative UDVA, CDVA, refractive, keratometric, topography and tomography data by Scheimpflug and OCT by 24months.

Results:

Mean values changes: UDVA from 20/40 to 20/16, CDVA 20/20 to 20/16, 2 eyes lost 1 line of CDVA, 7 eyes took over 5 days to re-epithelialize, 6 eyes developed transient corneal haze grade 2/4 resolving by month 12. 2 eyes were diagnosed with early cataract changes by month 12 and removed from the final data. All imaging data to include epithelial thickness map normality improved by month 3.

Conclusions:

tPRK retreatment after myopic L&S utilizing TMR appear to be safe and effective in improving visual function. Avoiding flap complications and further corneal biomechanical challenge may surpass minor intrinsic morbidity associated with PRK.
