Five-year results of microdistortions in Bowman's layer and intraocular scattering after small-incision lenticule extraction for myopia

Abstract Details

Purpose:

To investigate 5-year results in Bowman’s layer microdistortions and intraocular scattering after femtosecond laser small incision lenticule extraction for myopia.

Setting:

EENT Hospital of Fudan University

Methods:

In this prospective study, 30 eyes of 17 patients were treated with SMILE. Mean age was 28.86±5.24 years and mean spherical equivalent was -5.49±2.91 diopters. Examinations were performed preoperatively, 2 years and 5 years after SMILE. Bowman’s layer microdistortions were measured using Fourier-domain optical coherence tomography. The Objective Scatter Index (OSI) was examined with an objective optical quality analysis system.

Results:

Five years after SMILE, no eyes exhibited loss of corrected distance visual acuity. Microdistortions were observed in 68.97% of the eyes at 2 years and 53.57% at 5 years postoperatively (P<.05). The width of the microdistortions per meridian was 167.24±142.19 μm at 2 years, and decreased to 145.83±138.52 μm at 5 years (P<.05). No significant association was found between OSI values 5 years postoperatively and microdistortion-related parameters (P>.05). Regression analysis models revealed that higher preoperative spherical equivalent and thinner preoperative corneal thickness were associated with more microdistortions 5 years postoperatively (P<.05).

Conclusions:

Microdistortions in Bowman’s layer can remain 5 years after SMILE, but their lengths decrease over time. The microdistortions had no impact on long-term visual performance.
