Prevention of pseudophakic cystoid macular edema: comparison of two perioperative treatment strategies

Abstract Details

Purpose:

To compare the efficacy of two perioperative treatment strategies to prevent the cystoid macular edema (CME) after cataract surgery. The difference in incidence of pseudophakic CME was evaluated in patients treated with dexamethasone and antibiotics or prednisolone and diclofenac.

Setting:

Centre for Ophthalmology Düsseldorf, Germany

Methods:

Retrospective chart review of 2,392 patients who had undergone phacoemulsification in 2017 and 2018 at the Centre for Ophthalmology Düsseldorf, Germany. In 2017 1,183 patients were included who received a combined preparation of dexamethasone and antibiotics (Dexa-Gentamicin eye drops) for a four-week postoperative interval. In 2018 1,207 patients were included who received prednisolone (Predni-POS 1%) and diclofenac (Voltaren Ophtha) eye drops two days preoperatively and four weeks postoperatively. All patients diagnosed with a clinically significant pseudophakic CME within the first three months after cataract surgery were identified.

Results:

Twenty patients (24 eyes from a total of 1802 eyes) who underwent cataract surgery in 2017 and were treated with dexamethasone and antibiotics developed a pseudophakic CME (incidence: 1.33%). In 2018 nine patients (9 eyes from a total of 1796 eyes) were diagnosed with a pseudophakic CME after treatment with prednisolone and diclofenac (incidence: 0.50%). Adjustment of the treatment strategy to prednisolone and diclofenac highly significantly reduced the occurrence of pseudophakic CME (p = .00896, p < .01).

Conclusions:

The rate of pseudophakic CME was highly significantly lower using a pre- and postoperative medical treatment with prednisolone and diclofenac when compared to the treatment with dexamethasone and antibiotics after cataract surgery.
