Incidence of glaucoma following cataract surgery in paediatric population at a reference centre in Mexico

Purpose:
To collect data from clinical records of eyes with aphakia and pseudophakia in children; who later developed glaucoma as a complication of cataract surgery, and thus to determine its incidence, since there is no statistical information on this topic in the pediatric population in Mexico
Setting:
Retrospective, transversal, analytical descriptive.
Methods:
The medical records of patients diagnosed with Glaucoma associated with aphakia and pseudophakia were reviewed in children under 18 years of age, from January 2008 to June 2018, at the Institute of Ophthalmology "Fundación Conde de Valenciana". All eyes with this diagnosis were included, eyes without clinical evidence of glaucoma or suspected glaucoma prior to cataract surgery and eyes who underwent cataract surgery caused by different etiologies. Eyes diagnosed with congenital glaucoma, developmental glaucoma, glaucoma associated with anterior segment anomalies or other etiology prior to cataract surgery were excluded.
Results:
2983 records were reviewed, 285 cataract operated eyes in total, 62 of these in aphakia and 223 in pseudophakia. 55 patients met the inclusion criteria, with a total of 71 eyes. An absolute incidence of 24.9% was obtained, 79% for glaucoma associated with aphakia and 9.8% in eyes with pseudophakia. The average age was 7 years 8 months, 49 eyes with aphakia presented glaucoma (69.01%) and 22 eyes with pseudophakia (30.9%). There is a relative risk of presenting glaucoma by aphakia of 4.94 compared to pseudophakia, which is statistically significant.
Conclusions:
This study shows that aphakia has a higher risk for the development of glaucoma compared to pseudophakia, in the same way there is a higher risk and it is more severe in those children who develop it at a younger age. The Institute of Ophthalmology "Fundación Conde de Valenciana is a national reference center which would explain the high incidence and not precisely due to the surgeries performed in our hospital but because most of the subjects found were referred from other hospitals.
