Purpose:
To evaluate the correlation between nuclear density obtained from three dimensional (3D) and region of interest (ROI) modes of Pentacam Scheimpflug imaging and NO score obtained from LOCS III grading of cataract with phacodynamic parameters during cataract surgery in age related cataract in Indian eyes.
Setting:
Observational, cohort, prospective , observer-masked study conducted on 200 eyes of 194 patients attending the outpatient department of Shroff Eye Centre, Kailash Colony, New Delhi from January 2017 to December 2018
Methods:
Cataract grading for all patients was done by LOCS III classification and NO score was documented. Lens average and maximum nuclear density were measured by 3D and ROI modes of Pentacam Scheimpflug imaging software and PNS grade displayed in 3D mode was also noted. All patients underwent uneventful phacoemulsification by a single surgeon using stop and chop technique and cumulative dissipated energy (CDE), total ultrasound time and total fluid (BSS) used were noted at the end of nucleus emulsification.
Spearman correlation coefficient was used to assess association of various parameters. Bland-Altman plot was used to compare 3D and ROI modes.
Results:
A positive correlation was found between CDE and total ultrasound time with all cataract evaluation methods. NO score and 3D average density showed a stronger correlation with CDE (r=0.567, p<0.0001) and (r=0.557, p<0.0001) respectively and total USG time (r=0.476, p<0.0001) and (r=0.466, p<0.0001) respectively. A positive correlation was also seen between the fluid used and density obtained on both 3D and ROI modes, except with ROI average lens density (r=0.103, p=0.1454).
Conclusions:
LOCS III and Pentacam Scheimpflug imaging are comparable tools for cataract evaluation. Addition of Pentacam Scheimpflug imaging to routine pre-operative workup for cataract surgery can offer an objective approach to enhance the prediction of phacodynamic parameters during cataract surgery.
