Abstract Details

Purpose:

To compare the different indices of keratoconus detection in pediatric eyes using the rotating Scheimpflug camera (Oculyzer II, Pentacam HR) in order to show the sensitivity and specificity of each one of them.

Setting:

Al Watany Eye Hospital, Cairo, Egypt

Methods:

All retrievable data of significance from 469 normal right pediatric eyes and 29 eyes of pediatric keratoconus (KC) and forme fruste KC (FFKC), imaged by Oculyzer II between December 2013 and October 2018, including slit lamp examination, refraction, Scheimpflug images data were collected. The area under the receiver operating characteristic curve (AUROC) was calculated for different keratoconus detecting indices.

Results:

The mean (95% confidence interval) age was 13.6 (13.3-13.9) years in normal pediatric eyes and 15.1 (14.3-15.9) years in FFKC and KC pediatric eyes. The indices with AUROC more than 0.96 were Front Elevation (FE), Back Elevation (BE) at Best Fit Toric Ellipsoid (BFTE), BE, FE-BFTE, Index of height decentration (IHD), Keratoconus Index (KI), Ambrosio relational thickness maximum (ART-max), keratometry maximum (K-max), Topographic keratoconus classification (TKC) and ART-average (AURO= .983, .98, .979, .974, .973, .97, .968, .967, .965 and .963 respectively). The FE index with the highest AUROC had 100% sensitivity and 97.45% specificity at > 6 micron.

Conclusions:

The most useful rotating Scheimpflug imaging indices to detect keratoconus in pediatric eyes were elevation, curvature indices, and pachymetry indices similar to those used in adults. However, the best cut-off values may differ.
