Purpose:
To assess visual outcomes, defocus curve, contrast sensitivity and patient's satisfaction after bilateral implantation of TECNIS®Symfony Extended Range of Vision (ERV) IOLs versus two different mix-and-match approaches:combining the Symfony ERV IOL with the +3.25 D(ZLB00) or the +4.0 D(ZMB00) TECNIS multifocal 1-piece IOL.
Setting:
Prospective, randomized clinical study, Glory Eye Center, Seoul, Korea
Methods:
In this study, 110 eyes of 55 patients were allocated randomly into 3 groups, bilateral Symfony IOLs with micro-monovision (Group SYM), mix-and-match Symfony IOL with bifocal +3.25 D IOL (Group SYM+3.25), and with +4.0 D IOL (Group SYM+4.0). The target refractive error of non-dominant eyes in Group SYM was -0.75 D. Binocular uncorrected visual acuity in distant, intermediate, near distance (UDVA, UIVA, UNVA), manifest refraction, defocus curve, contrast sensitivity (Takagi contrast glaremeter CGT-2000), subjective questionnaires (photic phenomena, spectacle independence and patient satisfaction) were evaluated at 1 week, 1 month and 3 months postoperatively.
Results:
Binocular UDVA of three groups were similar and Group SYM+3.25 and SYM+4.0 showed better UNVA (p < -0.05). Defocus curve presented similar pattern in distant focus, Group SYM presented better intermediate vision (range: -1 D to -1.5 D). Binocular contrast sensitivity test of Group SYM showed lower logCS values under the condition of ‘Night with glare’ (p < -0.05). Group SYM had less halo value and higher subjective satisfaction value for intermediate vision but lower for near vision.
Conclusions:
All three groups provided good visual outcomes at all distances with a high percentage of spectacles independence after cataract surgery. Two mix-and-match groups presented better near visual acuity and contrast sensitivity in specific condition, though bilateral Symfony group presented better intermediate visual acuity and less halo.
