Comparison of the surgical outcomes between two femtosecond laser cataract platforms

Purpose:
To assess and compare the capsulotomy and clear corneal incision (CCI) outcomes, incidence of subconjunctival hemorrhage and intraoperative miosis between two femtosecond laser platforms of cataract surgery.
Setting:
Universal Eye Center, Zhong-Li, Taiwan.
Methods:
This retrospective study included 200 eyes of 200 patients (94 male and 106 female) aged 68.3 ± 10.3 years who had undergone femtosecond laser-assisted cataract surgery using either LenSx (Alcon Laboratories) (Group 1, N=100) or FEMTO LDV Z8 (Ziemer) (Group 2, N=100) laser platform. Intraop-erative completeness of the CCI and capsulotomies, incidence of subconjunctival hemorrhage and intraoperative miosis were evaluated and compared between the two groups. The completeness of CCI (main wound and side-port) was classified as ‘complete’ or ‘incomplete’. Likewise, the completeness of the capsulotomies was classified as ‘complete, ‘incomplete’, ‘with microadhesions’, or with ‘anterior/posterior tear’.
Results:
Complete free-floating capsulotomies were more in group 2 (93% vs 87% in Group 1) however, the difference was not statistically significant (p>0.05); capsulotomies with microadhesions were similar (7% each); proportions of incomplete capsulotomies were better in group 2 (0% vs 5% in group 1) and proportions of capsulotomies with tear in group 1 and 2 were 1% and 0% respectively. CCI outcomes were comparable between the two groups (p>0.05). Group 2 performed significantly better (<0.001) in the incidence of subconjunctival hemorrhage (1% vs 63% in Group 1) as well as intraoperative miosis (0% vs 19% in Group 1).
Conclusions:
Both femtosecond laser platforms demonstrated similar capsulotomy and CCI outcomes; however, incidences of subconjunctival hemorrhage and intraoperative miosis were significantly lower in Ziemer Z8 group than LenSx group.
