Safety and effectiveness of a novel femtosecond laser-assisted capsular marking system for the alignment of toric IOLs: a prospective study

Purpose:
To investigate the safety and effectiveness of a novel femtosecond laser-assisted capsular marking system for the alignment of toric IOLs during cataract surgery.
Setting:
Eyes of York Cataract & Laser Center, York, PA and East Valley Ophthalmology, Mesa, AZ
Methods:
This prospective, multicenter study enrolled 25 patients with age ≥ 18 years, stable regular keratometric cylinder ≥ 0.50 D, vector difference of ≤ 0.50 D in anterior corneal astigmatism as measured by the Cassini Corneal Analyzer (Cassini Technologies) compared with the Zeiss IOLMaster 700 (Carl Zeiss Meditec) or the Lenstar 900 (Haag-Streit), and predicted residual refractive astigmatism (RRA) ≤ 0.50. All eyes underwent femtosecond laser-assisted cataract surgery followed by toric IOL implantation. Alignment of toric IOLs was guided by capsular marks integrated into the femtosecond laser capsulotomy (IntelliAxis Refractive Capsulorhexis, LENSAR).
Results:
Preliminary data available at postoperative week 6 demonstrated mean MRSE, UDVA and CDVA of -0.02 ± 0.51 D, 0.06 ±0.20 logMAR and -0.01 ± 0.10 logMAR, respectively. Mean RRA measured -0.12 ± 0.19 D compared with preoperative keratometric astigmatism of 2.52 ±0.73 D (p<0.001); 100% of eyes demonstrated ≤ 0.50 D refractive astigmatism. Rotation of toric IOL from the intended alignment was assessed in reference to the capsular marks; no misalignment was observed in any of the eyes. No adverse events were reported.
Conclusions:
Capsular marks with LENSAR IntelliAxis Refractive Capsulorhexis were safe and effective for toric IOL alignment during cataract surgery. All eyes demonstrated RRA ≤ 0.50 D 6 weeks postoperative and no IOL misalignment was observed.
