Retrospective comparison of white-to-white measurements between the Meibox Meibographer and the Pentacam HR and IOLMaster: a pilot study

Abstract Details
Purpose:
To determine if the Meibox infra-red (IR) meibographer (Box Medical Solutions, West Hills,California, USA) produces white to white (WTW) values similar to established imaging devices (e.g.Pentacam HR [Oculus GmbH, Wetzlar, Germany] and IOL Master [Carl Zeiss Meditec AG, Jenna,Germany]) used during refractive cataract workup.
Setting:
Private 2 provider ophthalmology practice located in Upstate New York, United States.
Methods:
WTW values from previously recorded IR images captured in “pupil mode” were generated by a single observer via manual digital calipers on 16 individuals (32 eyes) presenting for pre-operative evaluation. Each image was read in triplicate and the average read utilized. Caliper lines were required to bisect the light reflection of the camera in the center of the pupil. Resultant values were compared against paired computer generated WTW values from the Pentacam HR and IOL Master 500. Exclusions: blurry or off-center images, individuals < 18 years or > 75 years. Data were analyzed in Microsoft Excel.
Results:
Average coefficient of variation between IR image reads was < 1% (Standard Error = 0.06 mm). After exclusions, n = 21 yielding average WTW values of 12.50 mm (+/- 0.59 mm), 11.93 mm (+/-0.55 mm), and 12.21 mm (+/- 0.47 mm), Meibox, Pentacam, and IOL Master, respectively. Mean fit differences were -0.572 mm Meibox/Pentacam (t= -3.67; p=0.0015) and -0.282 mm Meibox/IOL Master (t= -11.50; p<0.0001). A comparison of Pentacam/IOL Master yielded a mean fit difference of 0.29 mm (+/- 0.34 mm; t= 3.94; p=0.0008). Post hoc power = 0.99.
Conclusions:
In this single observer retrospective of IR images analyzed with digital calipers, the Meibox consistently returned W2W values statistically significantly higher than the Pentacam and the IOL Master. Therefore, the W2W values obtained from the Meibox were reproducible amongst a single observer, the resultant W2W values are not commensurate OR interchangeable with W2W values generated from the Pentacam or IOL master.
